Managing chronic conditions requires a good understanding of your own health conditions. In particular, what can be expected to happen, what you need to do to optimise your health and what you can do when you get sick.

We encourage our patients to undergo chronic disease management planning where appropriate and can work with you to get the most out of your body and mind.

Medicare Funding For Eligible Chronic Conditions

If you are living with a “chronic disease” you may be eligible for 3-4 bulk billed check ups and 5 Medicare subsidised visits to appropriate allied health providers such as an optometrist, podiatrist, dietitian, or physiotherapist if needed (gap payments may apply).

  • Heart disease and stroke
  • Lung disease including severe asthma, emphysema, chronic bronchitis
  • Diabetes
  • Arthritis
  • Osteoporosis
  • Depression/Anxiety

..and other conditions that are likely to or have lasted at least 6 months. Please speak with your doctor to find out if you are eligible.
